*Job Title: LPN* *Location: Canton, NY 13617* *Duration: 13 weeks with possible extend* *Schedule: Days- 12 hours * *Guaranteed Hours: 40 hours* *Duties:* · Seeking an LPN with experience in geriatric long-term care as well as acute rehabilitation care.
· Participate in the assessment process by collecting information and reporting to the Registered Nurse.
· Assist the Registered Nurse in the formulation of the meaningful plan of care.
· Provides information on patient progress to RN in order to have input into plan of care revision.
· Monitor patient nutrition and fluid balance.
· Provides emotional support to the patient and family.
